Facial fat grafting in midlife is not simply filling hollow areas. Some parts of the face lose volume while tissue in others moves downward, so hollowing, sagging and bulging can appear at the same time.
1. What is facial fat grafting?
Your own fat is harvested, purified and injected into areas lacking volume. The survival rate varies considerably with the area treated, the injection technique and the patient's condition.
2. How does fat grafting differ between younger and midlife faces?
In younger patients the goal is mainly to shape volume, whereas in midlife — where hollowing and sagging appear together — the goal is smoothing out the differences in height.
3. Hollowing and sagging must be assessed together
Hollow areas (temples, above and below the eyes, the anterior malar area) can be filled with fat, but tissue that has already descended (eyebrows, nasolabial folds, jowls) is hard to raise back to its original position with fat grafting alone.

Large weight loss can cause volume loss and skin laxity together. Areas commonly considered for midlife patients include hollowing above the eyes, the forehead and temples, under the eyes and the anterior cheeks, and the midface groove (the "Indian band").

Putting more fat than necessary into the outer cheeks, the outer cheekbones, the jowls or around the lower jaw can make the face look wide and heavy.
